Transcript & Record Requests

Copies of applications, transcripts, report cards and other school records are available by contacting the Pupil Personnel Services (PPS) office. Please use the Request for Copies form when making requests. Forms may be e-mailed or mailed to Lexington in care of the PPS Department, 25-26 75 th Street, East Elmhurst, NY 11370.
